The play
Require human approval for agents touching production infrastructure, and audit logs, permissions, and disclosure handling before expanding autonomous operations.
A blog post describing the incident says OpenAI’s automated agents had identified a caching vulnerability in RubyGems before it was publicly disclosed. The story drew 230 points and 213 comments on Hacker News, which tells you security engineers are paying attention.
The important point for business owners is not RubyGems itself, unless you run software built on it. It’s the wider pattern. AI agents can now inspect systems, connect dots across technical information, and potentially act faster than the people responsible for the infrastructure. That creates value for finding problems early. It also creates a new risk if an agent has access to sensitive systems, can make changes, or surfaces information before the right people have assessed it.
Most companies don’t need to stop using AI because of this. They do need clearer rules around what agents can see and do. Separate read access from write access. Keep approval steps for production changes. Log agent activity. Make sure someone owns the response when an agent flags a security issue, rather than assuming the tool has handled it.
This is the kind of thing we build into an AI command centre, giving leaders visibility over where AI is operating, what it can access, and when human review is required.
