The problem
After an incident the driver rings whoever answers, details are scribbled on a notepad, photos stay on a personal phone, and the incident record is written days later from memory. Near misses almost never get reported.
How it works
- Asks first whether anyone is hurt and whether the scene is safe, and tells the caller to ring 000 if anyone is hurt
- Takes the driver, truck and trailer numbers, location, what happened and photos
- Opens an incident record with a reference number
- Tells dispatch at once, and the compliance manager and managing director for any injury, rollover or load loss
- Accepts near miss reports anonymously if the driver wants
- Never decides who is at fault
What it connects to
- Kate (driver)Hi, it's Kate in rigid R-08. I had a pallet shift coming round the roundabout at Larapinta. Nobody's hurt.
- AgentGlad you're okay, Kate. Are you parked somewhere safe, and is the load stable now?
- Kate (driver)Yeah, I'm in the GreenLeaf yard. One pallet tipped against the side curtain, curtain's bulging but holding.
- AgentThanks. Please don't open that curtain until someone checks it. Can you text me two or three photos from outside? I've opened incident INC-1147 and told Kylie in dispatch. She's sending a forklift driver from GreenLeaf to help.
- Kate (driver)Sending now. Will this go on my record?
- AgentI just record what happened, I don't decide anything about fault. Leanne Cho in compliance reviews every incident and will talk it through with you.
